District Certification Forms and Information

READ BEFORE APPLYING:

 

  

  • A complete application consists of three parts:
      • Properly completed application. Sign after printing.
      • Payment (Money Order, Online Payment or Payroll Deduction (if applicable)
      • Supporting documents (see specifics below)

  

  • Incomplete applications will not be accepted.
  • Please print all forms on white paper ONLY. All others will be returned for correction.

  

  • Add-on and renewal applications can only processed for current District employees and charter school teachers. Retirees and persons not employed by the District must apply directly to DOE. (DOE will not accept District forms)

   

  • Submit completed applications (in person or by Pony) to:
    Certification Office, FHESC Suite A-152

  

District Add Subject or Endorsement Application

  

  • Do Not use this form if you hold a Temporary Certificate, or if adding Reading K-12, Speech-Language Impaired, School Psychologist,
    Ed Leadership or Guidance and Counseling.

  

  • Attach supporting documents for subject/endorsement being added (e.g. Subject Area Exam score report, Endorsement completion verification, official transcripts, etc.)

  

District Renewal Application

   

  • You may renew your certificate up to one year prior to its expiration date. Renewals for 2007-2012 certificates are now being accepted.

  

  • Requires a minimum of 120 inservice pts or equivalent credit (Renewal Requirements)
    Include inservice printout with your application.
