A Hospital/Homebound student is a student who has a medically diagnosed physical or psychiatric condition which is acute or catastrophic in nature, or a chronic illness, or a repeated intermittent illness due to persisting medical problem and which confines the student to home or hospital, and restricts activities for an extended period of time.  The annual medical diagnosis shall be made by a licensed physician (MD or DO) as defined in Chapters 458 and 459, Florida Statutes licensed to practice in the State of Florida.

                                             

A student, who is hospitalized or homebound, is eligible for specially designed instruction if the following criteria are met.  A licensed physician must certify that the student:

(1)  Is expected to be absent from school due to a physical or psychiatric conditionfor at least fifteen (15)

       consecutive school days or the equivalent on a block schedule, or due to  a chronic condition, for at

       least fifteen (15) school days or the equivalent on a block schedule, which need not run consecutively;

(2)  Is confined to hospital or home;

(3) Will be able to participate in and benefit from an instruction program;

(4) Is under medical care for illness or injury which is acute, catastrophic, or chronic in nature;

(5) Can receive instructional services without endangering the health and safety of the instructor or other

      students with whom the instructor may come in contact;

(6) The student is enrolled in a public school in kindergarten through twelfth grade prior to the referral for

      hospitalized or homebound services unless the student meets criteria for eligibility under Rules 6A- 

      6.0.011, 6A-6.03013, 6A-6.03015, 6A-6.03016, 6A-6.0318, 6A-6.0322, 6A-03023, 6A-6.03027, FAC;

(7) A parent/guardian or primary care giver signs parental agreement concerning hospitalized or

      homebound policies and parental cooperation.

 

PROCEDURES FOR DETERMINING ELIGIBILITY

Procedures for determining eligibility shall be in accordance with Rule 6A-6.0331, FAC.  The individual educational plan shall be developed or revised prior to assignment to the hospitalized or homebound program placement as required in Rule6A-6.03028, FAC.  A student may be alternatively assigned to the hospitalized or homebound program and to a school-based program due to an acute, chronic, or intermittent condition as certified by a licensed physician.  This decision shall be made by the Individual Educational Plan (IEP) in accordance with the requirement of Rule 6A-30328, FAC.  The IEP meeting for secondary students (grades 6 through 12, projected to be enrolled in the Teleclass program) will be held at the Hospital/Homebound Office.  IEP meetings for elementary students are usually held at the school.

 

DISMISSAL FROM THE PROGRAM 

Parents cannot bring their child back to the home school prior to the initial date listed on the

PBSD 0312 unless there is a release on file in the Hospital Homebound office.

A student will be dismissed from hospital homebound on the “Expected School Return Date” as indicated by the physician on the form PBSD 0312. If an extension to the original dismissal date was received from the diagnosing physician, the student will be dismissed on the new dismissal date.

 

Dismissal prior to the original dismissal date.  If a student is permitted to return to school prior to the original dismissal date on the PBSD 0312 form, the physician must fax a release to return to school to the Hospital Homebound office at 684-5214.  The release must be on the physician’s stationery or on a prescription form from the physician’s office.

 

A student may also be dismissed from the program for the following reasons: 

1. medical eligibility expires,

2. the student is no longer confined to the hospital or home,  

3. non-attendance,

4. excessive absences,

5. failure to keep scheduled appointments,

6. going on vacation,

7. if the student is employed or participates in extra-curricular activities.

Text books and materials.  Text books and all materials loaned to the student and the parents while the student is enrolled in the Hospital Homebound Program must be returned to the Hospital Homebound office before the student returns to school.

Employment and participation in extra-curricular activities while on Hospital Homebound. Working and participation in extra-curricular activities is not permitted during the time a student is enrolled in the Hospital/Homebound Program.  If a student is able to have a job, go to work and function in the workplace or participate in extra-curricular activities, he/she is able to attend his/her regular school.

 

Required forms. The forms listed below must be completed and submitted to the Hospital Homebound office for consideration of a student’s enrollment in the Hospital Homebound program.  Just click on the name of the form and you will be directed to the appropriate forms. Print out a copy of the Request for Consideration, 0312 form, compete Section A on the form then take it to the physician and have the physician complete all of Section B of the 0312 form.  When the form is completed by both the parent and the physician, fax the form to the Hospital Homebound office.  Next, print out a copy of the Parent and Student Agreement, 2101 form and fax the signed copy of the form to the Hospital Homebound office.
