All applicants must complete and sign this form as part of the hiring process. Original Security Check Forms should be submitted to Recruitment and Retention A-132 (No faxes or copies).
New applicants must submit a minimum of three references to complete the hiring process. References should reflect employment for the last 5 years and must be completed by supervisor(s). Applicants with limited work experience may supplement their work reference with personal references. References from relatives are not acceptable. If you are currently in school/college, you may provide references from professors/instructors.
Applicants completing student teaching must submit references from 1) clinical educator(s)/supervising teacher, 2) university supervisor, and 3) school assistant principal and/or principal.
References should include applicant id number, signature with date, of applicant and evaluator.
Additional Documents Needed for Employment
1) College Transcripts (Degree Conferred/Awarded), 2) Most Recent Evaluation (professional or student teaching), 3) Proof of Florida Certification, and 4) Verification of Highly Qualified Status (NCLB).
If you have professional teaching experience and would like it to be considered for compensation purposes, you must have each previous employer complete and submit the Verification of Previous Public/Private Teaching Experience form.
