Former students, age 18 years of age or older, must submit a written request before confidential information may be released regarding that student's records. The former student's parent or legal guardian may sign for the student if the student or former student is under age 18. To be fair to all persons who request student records, all requests must be treated on a first-come, first-serve basis. The Records Management staff does their very best to answer all requests as quickly as possible. All requests will be processed within five (5) working days after receipt of the request.

STUDENT TRANSCRIPT/ RECORDS REQUEST PROCEDURES

The Records Management Office processes transcripts/records for students who last attended Palm Beach County public school programs three (3) or more years ago or from schools that are closed. All other requests should be sent directly to the current/last school the student attends/attended. 

A former student's written signature is required before confidential information may be released regarding that student's records. Records requests are processed on a first-come, first-serve basis. Records Management staff does their best to answer requests as quickly as possible. Requests will be processed within five (5) working days after receipt of the completed request.

 

REQUESTS IN PERSON

Individuals requesting records in person must complete the "Student Transcript/ Records Request" form (PBSD 1457) and leave it with a School District staff member. The request will be processed and the records mailed or faxed to the desired location within five (5) working days after receipt of the request. 

 

MAILED REQUESTS 

Persons calling the Records Management Office to request student records will be told they must mail or fax a written request. Information that must be provided on the written request (PDF file) includes: 

1. Former student's full name (including maiden name, if applicable);

2. Last school attended in Palm Beach County;

3. Last year in school;

4. Date of birth;

5. Social security number;

6. Daytime telephone number;

7. Whether the former student graduated, withdrew or received a GED;

8. If the former student received a GED, the year he/she received it;

9. Purpose of request (employment, education or personal use)

10. Address or fax number to send the records to; and

11. The former student's current signature.

CLOSED NON-PUBLIC SCHOOLS  

When a non-public school in Palm Beach County closes, all permanent information contained in student records shall be transferred either to the:

(1) Superintendent of the Palm Beach County School District; or

(2) principal office of the non-public school system or the association which the non-public school was a member.

Reference F.S. 1002.42(3)(b) The Palm Beach County School District has student records from many closed non-public schools. For questions about a specific closed non-public school, contact the Records Management Office at 561-434-8029. 

 

GED RECORDS 

The Palm Beach County School District is a GED testing facility. However, school districts are not authorized to issue official GED transcripts. All requests for verification of GED test scores and official GED transcripts must be handled by the Florida Department of Education, GED Testing Office. (See PBSD 1457).  

To request an official GED transcript, a written request must be sent to:

IMMUNIZATION RECORDS

    • If you graduated or left the Palm Beach County School District before the 1991-92 school year, your immunizations records were classified as temporary records and were destroyed.
    • If you graduated or left the District during or after the 1991-92 school year, your immunization records will be maintained as permanent records.  Per current procedure, student records are maintained at the last school attended in Palm Beach County for three years before being transferred to Records Management.
    • If you graduated or left the District less than three years ago, please contact the Records Coordinator at the last school you attended to request your records.
    • If you graduated or left the Palm Beach County School District more than three years ago, your immunization records will have been transferred to Records Management at School District Headquarters.

 

"UNABLE TO LOCATE" RECORDS 

Occasionally, Records Management staff members are unable to locate a student's records for one or more of the following reasons:  

1. incomplete birth date information provided;

2. incomplete identification of last public school attended in Palm Beach County;

3. last year of school attendance not provided;

4. former student's signed authorization to release education records is not provided; and/or

5. no record under name as given.  

In each of the cases listed above, the person requesting the records will be contacted in an effort to obtain information necessary to process the records request. If a telephone call is unsuccessful, a letter will be sent to the person requesting the records explaining the reason(s) the request cannot be processed.
