
You can add a dependent to most of the benefit plans you enroll in. Eligibility requirement for plans may vary so we encourage you to review the dependent requirements and maximum age information for each plan independently before enrolling your dependent.
As you add your dependent to the online system please keep in mind that adding a dependent is a two step process and you are required to provide documentation that shows your dependent is eligible to enroll in our plans to complete this process:
- You must complete your enrollment and make sure that you enter social security information for each covered dependent AND provide required documents by the end of your election period
- New Hire/Rehires - within 30 calendar days
- Life Events (marriage/birth/divorce/etc) within 60 calendar days
- Annual Enrollment - by the last day of the enrollment period
To enroll a dependent in our medical, dental and/or vision plans during the Annual Enrollment period you should schedule and appointment with an Enrollment Counselor or you can mail or bring the documents to Risk & Benefits Management - A103 in the Fulton-Holland Education Service Center. Please review the Sample Dependent Audit Verification Guidelines to determine the appropriate documents that are required based upon your relationship with your dependent.
Documents to verify your relationship to your dependents must be provided to the Risk & Benefit Management department prior to the end of the enrollment period.
As a result of Health Care Reform, you are able to enroll married/unmarried children up to age 26. Please read about this and other important Federal Notice that become effective for our plans as of January 1, 2011.
