We are excited to provide your with a private and secure manner to complete your benefit enrollment. The steps are as easy as accessing your e-mail!  First-time users will need to create a password. You can then simply click on Log In at the top of the School District's Home Page, then Self Service from the District's Employee Portal.

For step-by-step enrollment instructions, click here! If you are enrolling your spouse, domestic partner or any children to you medical plan you will need to include the following information for each of them:

*Gender  *Date of Birth  *Social Security Number

If you do not have a Social Security number for a dependent, such as a newborn child or newly adopted child, please obtain one from the Social Security Administration.

New Employees/Rehired Employees

Medical Plan choice is limited for a minimum of 12 months:

New plan options are made available during the annual enrollment following a minimum of 12 month of active employment.


New Hires - Be sure to complete your Tobacco Affidavit in Self Service/Benefits/Wellness Rewards and Surcharge within 30 calendar days of your hire date/benefits eligibilty date.  Otherwise you are subject to a $50 per month Tobacco Surcharge.  Read more about this on the Wellness page.

Prescription Vendor - Need to sign up for Mail Order Prescriptions? Just download an OptumRx form found at www.myuhc.com.

Below is a summary that highlights the changes for our 2014 plan year.  For detailed plan information, you can read the user-friendly Frexible Benefits Reference Guide.  You can also link quickly to specific information by clicking any item in the Benefits Reference Guide's table of Contents.



  • Medical
    • New Consumer Driven Health Plan (CDHP) replaces the PPO Planl
    • Current PPO plan participants will be defaulted to the CDHP unless changes are elected.
    • CDHP rates are much lower than the existing PPO medical plan.
      • This is a high-deductible health plan that provides freedom and flexibility.
      • Nationwide Network - UnitedHealthcare Choice Plus
      • Annual deductible:  $4,550 Individual, $9,000 Family
      • Annual Out-of-Pocket Maximum:  $6,350 Individual, $12,700 Family.
      • Out-of-Network benefits available (high Annual Out-of-Pocket maximums apply)
      • NEW - Health Savings Account (HSA) available to CDHP enrollees
        • Special eligibility rules apply for HSA's
      • Deductibles, copays and coinsurance apply towards Out-of-Pocket Maximums
        • Prescription copays do not apply towards Out-of-Pocket Maximums for HMO plans.

  • Dental
    • Managed Care DHMO Plans
      • S500 PB - slight rate increaseName change to S700PB - No change to plan rates
      • S700 - Name change to S700PB - No change to plan rates
      • S700PB - All participants to receive new I.D. cards.
      • Both plans enhances to include up to 3 composite resin (white) fillings at zero copay.
      • Rider added for implant coverage (plan limitation may apply)
      •
    • PPO Plans
      • Rate increase applies to both PPO plans (High Option 5215 and Low Option 5105)
      • Coverage for Implants (plan limitation may apply)
  • Vision - Slight rate increase. SNAPSHOT OF VISION BENEFITS
  • Flexible Spending Accounts (Health and Dependent Care)
    • Transition to new claims platform (FBMC partner WageWorks)
    • New cards have been issued with WageWorks logo effective 1/1/2014.
  • Wellness Rewards Program
    • Program requirements remain the same for 2014.
    • Program requirements will change for 2015.
