Preventive care services include: Adult • Annual routine office visit and exam • Tetanus/Diphtheria booster • Annual influenza vaccination (flu shot) • Cholesterol screening • Annual mammogram after age 40 • Annual Pap smear and pelvic exam • Labs, pathology, chest X-ray, and EKG (when performed as preventive care) Child • Six visits 0 - 12 months • Three visits 12 - 24 months • Annual visits age 24 months through age 18 • Annual Pap smear and pelvic exam, as appropriate by age • Lead-level testing • Immunizations • Labs, pathology, chest X-ray, and EKG (when performed as preventive care) Preventive Care: Routine annual screenings to “prevent” illness or injury.
