Athletics Eligibility Packet Now Online

  • All athletic physical packets must be completed and uploaded onto a new website called Aktivate. Hard copies will no longer be accepted. This system is in effect for both middle and high school athletics. Parents and students can fill out the student and parent information on the online forms or print them out and complete them and then upload them onto the Aktivate website. The actual athletic physical paperwork must be printed, completed, and signed and stamped by the doctor and then uploaded onto the website. All athletes must have a sports physical completed. A birth certificate, for the student, is also required to be uploaded. Two of the papers MUST also be completed and signed in front of a notary public and then uploaded. Please do not forget to do so. Also, please make sure to fill out the insurance information for the student.

    For boy’s baseball and girls softball, all paperwork must be completed and approved before the student can play in a game. For the remaining sports, this year and in the future, the paperwork MUST be completed PRIOR to tryouts starting. When registering, please indicate all sports the student will be playing or trying out for. This way, you will only have to go through this process one time a year. You will not have to do it for each sport. Please do not wait until the last minute and get the paperwork completed before the tryouts begin. I have attached a tentative schedule of when each sport will begin this school year. There is an attached document to help you with registering your child on Aktivate and also a link to a video which helps you get through the process.

We can set up an appointment for you to come to school to get assistance. With uploading the documentation. We can offer assistance in Spanish and Creole also. 

    Another change this year is ticket sales. All students and athletes must download the GOFAN app or visit GoFan.Co and purchase tickets to middle and high school games on the app. A credit card must be attached to the account. For Conniston MS, we only charge for indoor home games, boys and girls volleyball and boys and girls basketball. All other home games are free at Conniston MS. Your child will let you know if and when other schools charge. When I get that information from the other schools, I will relay that information to the coach and student-athletes. Tickets will be $3 for adults and $2 for students/children for home, indoor games at Conniston MS.

    Lastly, per school district policy, when entering any Palm Beach School District School, including Conniston MS, you cannot bring in large bags or purses. Only wristlets are allowed. Diaper bags are subject to a search. Please leave large bags and purses in your vehicle or at home.
