YOU MUST HAVE AN EMAIL ADDRESS ON-FILE IN SIS IN ORDER TO RECEIVE YOUR SIS GATEWAY PIN VIA THIS REQUEST FORM.

This form is only for students of Palm Springs Community Middle School. If you have children enrolled at other School District of Palm Beach County schools, you will have to contact those schools directly for your SIS Gateway pin.
