District Certification Forms and Information
READ BEFORE APPLYING:
Remember you may only renew when:
- You have earned 120 in-service points, which includes 20 in ESE, and
- You are in the last year of your certificate’s validity - i.e. your cert is 7/1/2014 to 6/30/2019, you can renew any time after 7/1/2018
STEP 1 - Go to the FLDOE Certification website and apply for your renewal. NO MONEY should be charged at this step for District employees. You are required to register on the website first (instructions) then apply to renew your certificate.
STEP 2 – Submit complete certification renewal verification form, payment and renewal requirements to the Certification Office, FHESC, Suite A-152.
Step 3 - Within three to four weeks after your completed application is received by the certification office, you should receive an e-mail from the FLDOE that your NEW CERTIFICATE is ready for you to print.
- You may renew your certificate up to one year prior to its expiration date on or after July 1.
- Verify a minimum of 120 in-service points or equivalent credit (Renewal Requirements)
Include in-service printout with your application.
