Online Registration Process for Student Athletes

Prepare to “Aktivate!” New Online Registration Process for Student Athletes Launches in 2022-2023 School Year

All student-athletes in grades 6 through 12 will register their physical packet and other required eligibility components into a new online system called Aktivate, beginning with the 2022-2023 school year. Paper athletic packets will no longer be accepted at school sites.

The online platform, formerly known as Register My Athlete, will be rebranded as Aktivate Registration on July 30, 2022.

High school student-athletes will need to electronically complete, or digitally upload the following documents and certificates into the registration system:

FHSAA EL2 – Participation Physical
FHSAA EL3 – Consent and Release
PBSD Form 1588HS – Athletic Eligibility for High School Students
PBSD Form 1589HS – Student Medical Consent for Athletics
PBSD Form 2608 – Interscholastic Athletics Accident Insurance
• NFHS Certificate – Concussion For Students
• NFHS Certificate – Heat Illness
• NFHS Certificate – Sudden Cardiac Arrest
• Birth Certificate (for proof of age purposes only)
• Receipt – Purchase of Interscholastic Athletics Accident Insurance

Middle school student-athletes will need to electronically complete, or digitally upload the following documents and certificates into the registration system:

FHSAA EL2 – Participation Physical
FHSAA EL3 – Consent and Release
PBSD Form 1588HS – Athletic Eligibility for High School Students
PBSD Form 1589HS – Student Medical Consent for Athletics
PBSD Form 2608 – Interscholastic Athletics Accident Insurance
• Birth Certificate (for proof of age purposes only)
• Proof of Health Insurance

Please note that Athletic Directors may have other school-specific documents linked into the system that may need to be completed. The system includes buttons which will link to all required forms and courses. Parents should follow all directions and prompts within the system to complete the process correctly.

The District’s coaches, both paid and volunteer, instructional and non-instructional will also be required to upload certain documentation into the Aktivate system. Coaches should work with their Athletic Director to begin the registration process. Coaches will need to electronically complete, or digitally upload the following documents and certificates into the registration system:

• CPR/AED/First Aid Card or Certification
• FLDOE Temporary or Five Year Coach Certification (not required for faculty).
• NFHS Certificate – Concussion In Sports
• NFHS Certificate – Heat Illness
• NFHS Certificate – Sudden Cardiac Arrest
• FHSAA Policy 36 Recruitment Video Certification
PBSD Form 2670 – Athletic Coach Acknowledgment Form(s).
• Copy of School District Badge
• NFHS Certificate – Cheer and Dance Safety Certification (Sideline and Competitive Cheer Coaches only, required for all cheer coaches)
• NFHS Certificate – Football Tackling Elective Course Certification (Football Coaches only, required for all football coaches)
