Hard Copy Applications
Hard copy applications will be accepted January 26, 2020. Late applications may be processed after the lottery is conducted; depending upon the availability of seats in the desired program. To submit a late, hard copy application, click on the "Choice Programs Application" link, below. Type or print legibly in blue or black ink and return the completed application to: Department of Choice and Career Options, 3300 Forest Hill Blvd., C-124, West Palm Beach, FL 33406.
