The Palm Beach County Health Department’s Immunization Van will be at the School District of Palm Beach County’s Welcome Center on Wednesdays this summer to help students prepare for the 2021-2022 school year.

The Immunization Van will offer free immunizations for children two months of age through 12th grade on Wednesdays, from June 16 through June 30, 10 a.m. to 1 p.m. each day, at the Welcome Center in Building E behind the School District’s main office at 3300 Forest Hill Blvd. in West Palm Beach.

Students entering kindergarten, seventh grade, or registering for school in Palm Beach County for the first time must receive vaccinations and provide a Florida Certification of Immunization (DH 680 Form) to the school upon registration.

A Florida Certification of Immunization DH 680 Form indicating compliance with the current required schedule of immunizations must be presented prior to admittance and/or attendance in school.

The vaccinations listed below are required for students entering kindergarten or any grade as a new student in the County:

• Diphtheria/Tetanus/Pertussis (DTaP Series) – Five doses.
Hepatitis B (Hep B Series) – Three doses.
Polio (OPV or IPV) – Four doses (the last one after age 4).
Measles/Mumps/Rubella (MMR) – Two doses.
Varicella – Two doses or a provider verification of Chickenpox disease history on the DH 680 Form.

In addition to the K-12 vaccines, students entering seventh grade must have:

• Tetanus/Diphtheria/Pertussis booster (Tdap) – One dose.

If a student is currently enrolled in a Palm Beach County District-operated school and entering the seventh grade, an updated DH 680 Form must be provided prior to the start of school. Families should bring their child's immunization records to receive necessary immunizations.

Please note that the COVID-19 vaccine is not being offered through the Immunization Van. It is being administered throughout the summer at four School District locations. The COVID vaccine is NOT required for school registration.
