Additional Forms

Home Education students have the option of participating in athletics and dual enrollment/early admission. If a students wishes to pursue either of these options please complete the parent and student information on the applicable form(s). Be sure to provide parent and/or student signature if requested on the form. Submit the complete form via email to homeed@palmbeachschools.org. We will sign the form for active and compliant students and return via email.
