Immunization Records

If you graduated or left the Palm Beach County School District before the 1991-1992 school year, your immunization records were classified as temporary records and were destroyed.

If you graduated or left the District during or after the 1991-1992 school year, your immunization records will be maintained as permanent records. Per procedure, student records are maintained at the last school attended in Palm Beach County for five years before being transferred to Records Management. If you graduated or left the District less than five years ago, please contact the Records Coordinator at the last school you attended to request your records.

If you graduated or left the Palm Beach County School District more than five years ago, your immunization records will have been transferred to Records Management at School District Headquarters.

In-person requests are BY APPOINTMENT ONLY. Visit the Appointment Calendar to view available dates and times, and to schedule your visit.  

We encourage our customers to save time and take advantage of online services by visiting Parchment.

NOTE: Students under 18 years of age MUST be accompanied by a parent or legal guardian in order to receive school records/transcripts.

Students CURRENTLY ENROLLED in a Palm Beach County School must go directly to their school since jurisdiction of records has not been transferred to the District.

Request via Email, Mail, or Fax

Request must include the following:

  1. The Student Records Request form 1457 filled out, signed, and dated. Typed signatures are not accepted. Provide either a written signature or an e-signature using DocuSign.
  2. Copy of pictured ID of adult student, or parent/guardian if the student is under 18.

NOTE: Our office processing time can be up to 5-7 business days (this does not include the mailing time).

March 28, 2025 - Spring Holiday
May 26, 2025 - Memorial Day
