  1. Print the Provider/Physician Results Form. Click here for directions to get to the form.
  2. Bring the form to your physician or a Convenience Care Clinic.
  3. Fax the completed form to the number on the form. Within 48 hours you will receive an email from Quest, notifying you that the form has been received. Within 10 days, your Rally percentage bar will reflect the biometric screening.

Tobacco Surcharge

Tobacco is considered a leading preventable risk factor for illness and disease. The School District approved the surcharge as a financial incentive to motivate behavior change. A $50/month surcharge will be added to your monthly premiums if you use tobacco or do not complete the affidavit.

Step 1 Remember to review your current tobacco status (employees only) through the School District People-Soft Self Service.

You do not need to update your status each year, you just need to review and verify you still have the current tobacco status shown in our system between January 1 and October 15 each year. Your status on file will be enforced for the full plan year unless you complete and submit a Tobacco Use Change Notification Form.
