2018 Health Rewards

Biometric Screening


One of the first steps toward better health is to know our key numbers. Biometric screenings include:

  • Body Mass Index (Height/Weight)
  • Blood Pressure
  • Cholesterol Levels (Total and LDL)
  • Fasting Glucose (Blood Sugar)

2018 Biometric Screening & Physician Results Form: Click here for instructions on how to schedule an onsite screening or download and print the Physician Results Form for use at Physician's Office or Convenience Care Clinic. Once you get to the Quest site, you will need to answer one tobacco question before proceeding.

Note: If you are getting blood work through your Physician's Office, be sure that they send your blood work to LabCorp or other in-network lab. Quest is only processing these forms.

Note: If you registered for an onsite appointment but could not attend, call 877-818-5826 to reschedule or to print the Physician Results Form. If you choose to use the form, we recommend you fax it in yourself.

Health Survey


The Health Survey is a confidential, online interactive tool which allows you to assess your lifestyle habits that are directly linked to health status and health care costs. Click here to watch a short video to help you better understand the power of Rally!

  • Log into www.myuhc.com
  • Go to Health Resources and go to "Rally"
  • The Health Survey might automatically pop up. If not, from your Rally Dashboard, click "View Details" under Employer Rewards.
  • Scroll to available activities and complete Rally Health Survey if activity is not marked as complete.
  • Takes about 15 minutes to complete.


2018 Health Rewards with Outcomes

Check your status. Reach 100%. Next Deadline is August 31st.

As in the past, Health Rewards is a voluntary program available to all employees and their covered spouse/domestic partner who are enrolled in the District's medical plan. By completing a series of health actions, you receive an incentive to save up to $600 annually on your premium contribution in 2018.
Full Credit ($600) Deadline: August 31, 2018
Partial Credit ($350) Deadline: December 31, 2018

Rally Missions: Through Rally, you can complete 3 missions to help you stay accountable and on track with your health goals. Choose from 52 different missions that will help you move more, eat better, feel good and take care of yourself. Download the Rally App for easy access.

Duration: 4 weeks/mission; missions can be completed simultaneously


Telephonic Coaching: Work one on one with a coach to set goals, make a plan, and stay on track. Find personalized support and motivation to move more, lose weight, eat better, relax, manage a condition or quit tobacco.

Duration: 8-12 weeks

Real Appeal: Virtual, year-long weight loss and healthy lifestyle program. Meet weekly with a coach via smart phone, computer or tablet, and receive 24/7 support and tracking tools.

Duration: 16 weeks

Tobacco Surcharge

Tobacco is considered a leading preventable risk factor for illness and disease. The School District approved the surcharge as a financial incentive to motivate behavior change. A $50/month surcharge will be added to your monthly premiums if you use tobacco or do not complete the affidavit.

Step 1 Remember to review your current tobacco status (employees only) through the School District People-Soft Self Service. 

You do not need to update your status each year, you just need to review and verify you still have the current tobacco status shown in our system between January 1 and October 15 each year. Your status on file will be enforced for the full plan year unless you complete and submit a Tobacco Use Change Notification Form.
