Information From the School Nurse
My name is Lauren Lemon. I am the school nurse at Binks Forest Elementary. The clinic hours are from 7:00 a.m. - 2:00 p.m. I work on the days that the students are present. In the event that I am absent and another school nurse is not available, there is a trained designee to cover the clinic as well as respond to emergencies that may arise.
As the school nurse, I care for major and minor illnesses and injuries that occur at school. In addition, I administer medications; provide care and support for students with chronic and high-risk conditions such as diabetes, asthma, allergies, and epilepsy. I serve as a resource for parents, teachers, and students.

Medications at school require a physician authorization form from the MD’s office. It is the parent’s responsibility to have the physician complete the physician authorization form and bring it to the clinic. If the parent is unable to bring it, the physician may fax the Palm Beach County medication form to the school (Fax # 561-268-5013). Parents will be called as soon as the form is received from the physician. At that time, you will need to come into the clinic and sign the form, bring in the medication, and a small picture of your child for our records. All medications, either prescription or over-the- counter must have a current physician authorization form. Parents are required to bring the medication to the clinic. Please do not send any medication to school with your child. All medications, including over-the-counter products, are subject to this requirement.
In elementary school, vision/hearing and BMI screenings will be provided for certain age groups. If you do not want your child to participate in the screening programs, please select “NO” to the screenings on the registration form.
The physical form (form 3040) must be completed by the physician for all kindergarten and new students. The parent is required to complete the back of this form. The immunization form (form 680) is completed by the physician and brought to the school. The registration form contains a section to note medical concerns and allergies.
