If there is a court order barring either parent from removing the student from school or contacting the student, we need a copy of the court order.

If the student is taking medication, a permission form from your child’s physician must be completed and on file at the school.

• Physical examination is given within a year of enrollment, form DH3040
• Immunization record on form DH680
• All students are required to have completed:
  • Hepatitis B
  • Polio
  • DTP/DT series
  • Two MMRs
  • Two Varicella Vaccines
• Additionally, students entering the 7th or 8th grade must have tetanus/Diphtheria/Pertussis (Tdap) booster.
• Standardized Test Scores
• Birth Certificate
• Withdrawal form from the previous school

Upon initial enrollment in a school, a student or parent must produce two current documents reflecting the correct residential street address. Post office boxes, private mailbox addresses, or commercial establishment addresses are insufficient.

Examples of acceptable documents reflecting residential street addresses are as follows: (According to School Board Policy 5.011)

• Home telephone or cellular telephone bill
• Electric bill
• Rent receipt with the name of the tenant and landlord and contact information for all parties.
• The lease agreement with the tenant's name and landlord and contact information for all parties.
  • Valid only through the expiration date indicated on the contract.
  • Upon expiration, parents must present a copy of the renewed or new agreement to the school office.
• Mortgage
• A home purchase contract including a specific closing date, with a copy of the deed to be provided within 30 days of the closing date.
• State of Florida driver's license
• State of Florida identification card
• Automobile insurance policy
• Credit card statement
• United States Postal Service confirmation of address change.
• Evidence of correspondence, including a stamped, addressed, postmarked envelope delivered to the home address.
• Declaration of Domicile form from the County Records Department.
