Dear Parent/Guardian,
Welcome to West Boynton Middle School!
In the links below, you will find the registration packet you must complete to be registered at our school.
Be sure to read everything, complete the paperwork, and try not to leave anything blank. You must provide two proofs of your address; further information is included in the packet. For incoming 7th graders, it is REQUIRED that students are current with a physical exam and immunizations. Be sure to do this in August, as you want enough time to take care of this.
Once you have all your paperwork completed, including your proofs of address, please email them to gladys.duenas@palmbeachschools.org OR, call to schedule an appointment to drop off the documents OR, send them via regular U.S. mail to our temporary address: West Boynton Middle School / Attention: Gladys Duenas / Villa 1 8650 Boynton Beach Blvd. Boynton Beach, FL 33472 561-359-6900

In – County Transfers
- Completed registration form PBSD_0636
- Proof of residence
- Refer to the Proof of Residence section
- Last or most recent report card and standardized test scores.
- Withdrawal Form from prior school
- This does not apply if the child registers at the beginning of the school year.
If there is a court order barring either parent from removing the student from school or contacting the student, we need a copy of the court order.
If the student is taking medication, a permission form from your child’s physician must be completed and on file at the school.

Initial Entry Into Palm Beach County School District
- Proof of residence
- Refer to the Proof of Residence section
- Physical examination is given within a year of enrollment, form DH3040
- Immunization record on form DH680
- All students are required to have completed:
- Hepatitis B
- Polio
- DTP/DT series
- Two MMRs
- Two Varicella Vaccines
- Additionally, students entering the 7th or 8th grade must have tetanus/Diphtheria/Pertussis (Tdap) booster.
- Standardized Test Scores
- Birth Certificate
- Withdrawal form from the previous school
- This does not apply if the student registers at the beginning of the school year.

Proof of Residence
Upon initial enrollment in a school, a student or parent must produce two current documents reflecting the correct residential street address. Post office boxes, private mailbox addresses, or commercial establishment addresses are insufficient.
Examples of acceptable documents reflecting residential street addresses are as follows: (According to School Board Policy 5.011)
- Home telephone or cellular telephone bill
- Electric bill
- Rent receipt with the name of the tenant and landlord and contact information for all parties.
- The lease agreement with the tenant's name and landlord and contact information for all parties.
- Valid only through the expiration date indicated on the contract.
- Upon expiration, parents must present a copy of the renewed or new agreement to the school office.
- Mortgage
- A home purchase contract including a specific closing date, with a copy of the deed to be provided within 30 days of the closing date.
- State of Florida driver's license
- State of Florida identification card
- Automobile insurance policy
- Credit card statement
- United States Postal Service confirmation of address change.
- Evidence of correspondence, including a stamped, addressed, postmarked envelope delivered to the home address.
- Declaration of Domicile form from the County Records Department.
