Service Dog Fear/Allergy Form

As the school year progresses, there may be opportunities for service animals to be on our campus. We need to know if any students have allergies or fears relating to dogs. If there is a concern, please let us know how serious the allergy or fear is so that we can make sure that all students have a safe learning environment.
