FY23 Administration Information
The District School Effectiveness Questionnaire (SEQ) is an essential tool to provide feedback and support toward developing and maintaining a positive and supportive school culture and climate. The District uses the SEQ results to ensure continuous improvement in alignment with Strategic Plan Theme: High-Performance Culture) and to meet accreditation requirements. There is a SEQ Resource page with strategies for schools to use to improve their school’s effectiveness.
Our stakeholder responses are important! Ongoing research reveals that student/parent engagement in schools can improve student achievement, reduce absenteeism, and improve parent confidence in their child’s education. Students with involved parents or other caregivers often earn higher grades and test scores, have better social skills, and show improved behavior. In addition, by engaging teachers and staff, the school community can create an environment where all members are welcomed, supported, and feel safe in school.
Please use these flyers to increase awareness and participation. Flyers are available in English, Spanish, Portuguese, and Haitian Creole for parents and students. The flyer for staff is available in English.Administration Dates

Please Note for FY23:
- If possible, staff should use January 3, 2023 to take the SEQ.
- Everyone will take the survey online with paper options available, if needed.
- The student survey will include students in grades 3–12. Students should take the SEQ during SEQ WEEK (Jan. 30 - Feb. 3) with makeups from Feb. 6-10.
- Staff and students will use an SEQ tile in their portals.
- Parents will use a link accessible from:
- QR code on the flyers
- Link from the school's website
- Email or text message sent from Communications
- The survey items have been reduced to approximately 1/3 from years past.

School Response Rates

Please note, response rates are approximate and the final response rates will be reported in the analysis in the published Executive Summary.
You can help to improve your school's response rates by sharing the SEQ flyers with parents, posting the student and staff flyers around campus, and providing an opportunity for parents to use school computers before and after school in order to take the SEQ. -
Administration
Staff Version (English)
The Staff version includes all school-based employees (e.g., teachers, instructional, non-instructional, and school-based administrators). All school-based employees will see the SEQ tile in their Portals on January 3rd and if possible use that day to take the SEQ. Once the survey is completed and submitted, the survey cannot be taken again. Please allow staff time to complete the survey. The SEQ may take 5-10 minutes to complete.
Parent Version - Grades K-12 (English, Spanish, Portuguese, and Haitian Creole)Please encourage all parents to take the survey online. Schools may want to provide parents with opportunities to use school computers at designated times. For special requests, schools may print out the SEQ questions and use the paper option.
Student Version – Grades 3-12 (English, Spanish, Portuguese, and Haitian Creole)All students in grades 3-12 will use the SEQ tile in their Portals.

Paper-Based Option
There are TWO items required to take the SEQ on paper:
1. The SEQ Performance Matters Answer Sheet (Bubble Sheet)
and
2. The printed questions (see links below)
(The answer sheets must be scanned into Performance Matters by Monday, February 13, 2023.)

Who can take the SEQ on paper?Parents may take the SEQ on paper (if requested).
Students who have paper-based testing accommodations may take the SEQ on paper.
There is no paper option for employees.
Please note:
There are 200 SEQ pseudo IDs that have been added to each school for the paper-based SEQ option. These IDs were added to grade 13 (regardless of school level) so they would not get confused with any of the local or state assessment reporting in PM. After the SEQ window we will remove them from the system. These will produce anonymous answer sheets that only tie the responses back to the schools.
The 200 SEQ pseudo IDs begin with 3001#### (####=school number) and end with 3200####.
Each ID can be used one time for each SEQ Survey. Once for any student and once for any parent. (Student and parent do not need to be linked.)Do NOT make multiple copies of a single answer sheet.

When will we receive the Scantrons?
Gosh, we haven't used Scantrons in years! -
Which students are supposed to take the SEQ?
Students in grades 3-12 should take the SEQ. -
Will we receive an email with a link for the survey?
No. The survey links are located in the District Portal. Search "SEQ" to find the SEQ tile. -
How do parents take the SEQ?
Parents should click the link in the text message or email sent from Communications. They can also click the link on their school's website. -
How do we box up the paper-based surveys? Are there labels?
There are no surveys to box up. Paper-based surveys must be scanned in PM. -
Where are the language overlays?
There are no overlays as the students, parents, and staff can take the survey in any of four languages online. -
